  • Additionally, if you are able to get your contribution matched, a fundraiser can get credit on their fundraising page if you complete this form. 
  • Please include your donation details so we can ensure your matching gift, DAF grant, or check is properly credited to the participant’s fundraising page.
  • After submitting the form, be sure to complete the process through your company’s matching portal, your DAF administrator, or by mailing your check.
